What's the correct way to fill in the address for international cards? I know the lines have to match, but they don't have "states" and ship codes. I typically just filled in the info like it didn't have a state or zip and just put each line on the form to match each line of the card.

What about 1-800 numbers. Do I just drop the 1 and mark it as...

1) Can I ask how to put 11 digit non-US number like when you can only put 10 digit on the Other Category.
