It was basically an attention check. At the end of my survey they had a 5 digit number and then they gave me 4 options and I had to click on the same 5 digit number.

Yeah I don't have any idea if it was supposed to be the code or not but since the survey ended on the very next screen without any code I thought "well maybe that 5 digit number was supposed to be the code." As confusing as it was the requester will probably get a bunch of inquiries which hopefully once they see how confusing it was everyone will just be automatically approved.
