I have this issue once in awhile that I get errors when I do surveys in Qualtrics. If that's your issue, you need to clear the cookies time to time. Enter chrome://settings/siteData in your browser then type qualtrics. It will list all the qualtric surveys you've done. Click 'remove all shown' then 'clear all'. Should work again.

 

Hoping it's shorter than they say. It was 11 min. Definitely not fun. Plenty of writing.
