    What if there is a name mismatch? The HIT says Dr Glen L Mcmillan, the web site says David G. McMillan, DDS.

    I encountered a doctor that isn't listed, just wrote 'wrong doctor'.
    Maybe add a comment box?

    What if it says the dentist graduated from university A and university B and not clear which one is the school dentistry? Should I mark n/a?

    Past President of the Upper Pinellas County Dental Association and Delegate to the Florida Dental Association and West Coast District Dental Association. He currently serves on the Program Committee of the West Coast District Dental Association.

    To show what is past and what is current, would i paste all of that into the societies box... or how would you prefer it?

    What if it says DDS from university X and master’s degree in orthodontics from university Y .
    Should I enter both University

    I think it would be University X. Double-check the instructions, I'm pretty sure it's addressed there.
