I don't know if this will help you out or not, but on FF there is a greasemonkey script that shows all the code for whatever page you are on. I think it is called Firebug.
I just tped all my answers over into a word document so I can just enter them in quickly if i have to reload the page. should take me like 5 minutes to copy them all over.
I GOT IT. I'm on Chrome and had to edit the HTML to make it work. Typing out an explanation tutorial now.
If you're on Chrome, right click near the Submit button and click "Inspect Element". Scroll down in the HTML until you see the submit button code which looks something like this: <input type="submit" blah blah> Right click, edit as HTML and copy it (Control + C on keyboard). Scroll up a little bit in the HTML field and right click the tag that says </form> and click "Edit HTML" You'll get a large editable box. Go to the end of the box, and RIGHT BEFORE the </form> tag, paste the Submit button code (Control + V on keyboard) and press Enter. *DO NOT REFRESH THE PAGE* Press submit on the survey and it'll go through.
To explain, what happened was, the person who made that form put the submit button OUTSIDE of the actual form code, which renders the submit button useless. The fix is to edit the HTML to put the submit button INSIDE the form code to register the button to submit THAT form. If anyone needs further explanation, let me know. If you have Firefox, I'm not sure if Firebug does the same thing, but...won't hurt to try!
bahahahahahah beat me to it you can do it on firefox, mine just went through, though i deleted the extra nested form instead of moving the button, since techincally the button IS inside the correct form, it's just that the nested one makes the document think the form ends earlier than it does actually if you move the button shit the answers might not go through i think you might be better off deleting the small form that has 'javascipt return validate this' and no content except for   i think by moving the button what you're actually doing is just putting it before the first </form> tag, but the radio buttons with your answers are before the second </form> tag so it's submitting no values

Firebug is a Firefox addon that allows you to edit a live webpage in HTML. I have no experience with it, although I have heard of it, so I can't comment on it personally.
