I normally just click on the "Report your experience..." link on the TO extension drop down in my browser: http://i.imgur.com/Tluu8.png (Link since I think images are disabled)
When you are on Amazon mTurk, if you have the script, just hover over the requester name the way you would to see their general TO, when you do that there should be a link that says report your experience. That is the easiest way I've found.

I chose to stop doing those after a few because the word was "snow" and there was one pic that had a speck of snow on a distant mountain. I wasn't sure and there seemed to be a lot like that. I figured, better safe than sorry, you know? I just made the choice to pass on those.
