I have this issue once in awhile that I get errors when I do surveys in Qualtrics. If that's your issue, you need to clear the cookies time to time. Enter chrome://settings/siteData in your browser then type qualtrics. It will list all the qualtric surveys you've done. Click 'remove all shown' then 'clear all'. Should work again.
