What I did was look in the source for the Game Survey hit, then do a find for qualtrics in the source, copy from "https://" to "MID=", then put your worker ID at the end of the URL.
I emailed them asking for access and they gave me a password no need for a CC. Maybe they changed things.
Unfortunately, it was just me trying to make a joke with a novelty account that I made while watching Kitchen Nightmares. It said that I had made a duplicate account, but that account was made with a different email address than the one that I am posting with, so, I don't know, maybe I was just spamming or something, and "they" didn't like that.

And Lori - when this went down, you stated you sat in the ER for 5 hours with a child that could breathe. Please post the name of the ER. Before my local ER had a separate pediatric wing, my kids always took precedence. For anything. They always took the kids first. I would like to know the name of the ER that made you wait 5 hours with a kid that you said 'couldn't breathe'
