I think these guys just pass my mturk ID around. I have a complete HIT database. I've never done anything for this requester. And yet... I have that "Exc: [19-2446]" qual... sigh.
I just looked at the thread you linked in your signature. Have you made your goal for all 268? (if my count is correct) days in a row? Going off the Dec. 14th date in your picture there.
I've had that happen to me before too. I'm sure some of these requesters are connected (same school, etc.) and sometimes I do HITs that are multiple surveys, so maybe that's how we get those weird quals.
Stuff like this is why it should be the burden of the requester to prevent retakes, or at the very least, provide a list of IDs who shouldn't retake something. Even if we try to shoulder the burden, there's no way we can know if we've done something even if we keep meticulous records should a requester open a new account, a Lab opens a new account that previously had independent researcher accounts, someone who did research under an old account posts a new study under a Lab account, etc. I feel like since I've been here I've seen the rare case where a bunch of people receive rejections for repeating something with no way of knowing they did it before, but I'm honestly surprised it doesn't happen with the amount of surveys taken over time.
